Possible causes

Broken or loose radiator hose
Leaking water pump
Cylinder head gasket leak
Damaged expansion tank cap

Recommended actions

Check level in the expansion tank
Add antifreeze/water mixture
Perform system pressure test
Tighten hose clamps
